Vorkosigan again

This concerns a one-off exchange near the beginning of Captain Vorpatril's Alliance. It was not sufficient to send me flailing for the eject button, but is the sort of thing that I would warn any person who is trans* or knows trans* people about, even if they did not wish to go into the book spoiled.
Ivan is still conflicted about Dono's transition, and makes an entirely gratuitous crack to Byerly about it. Dono does not feature in this book's plot. (Byerly does.) Yes, the whole plot with Dono's transition was pretty central to how the reader met By, and clearly Ivan's still got issues, but that bit could have been entirely left out and -- I want to say "not affected the book", but in fact it would have not affected it so much that it would have improved.
